Throughout the College Counseling Process, students and families will be completing a wide array of forms for our offices and for college and university admission offices. This page contains downloadable versions of the forms most frequently used by our students when submitting materials to the Office of College Counseling.
*NOTE: The Letter of Summary and Appraisal is the letter written jointly by your College Counselor and Advisor on behalf of Saint Louis Priory. It is submitted with all applications. It does not replace Teacher Recommendations. Typically if a school requires Teacher Recommendations, you will submit 2 Teacher Recommendations AND the Letter of Summary and Appraisal.
